When to use Forms At A Glance

Authorization for the Release of Student Information Form (Appendix I)
  1. The District requests parent/guardian release before disclosing personally identifiable information to non-custodial parent/guardians. In the case of divorce, this is not needed if a legally binding document declares differently such as a divorce decree explaining parental rights.
  2. A parents/guardian of the student completes AUTHORIZATION FOR RELEASE OF INFORMATION ON STUDENT Form. Both parent/guardians signatures are required on the completed form; however a building administrator can sing off if one of the parent/guardian is unavailable for signature.
  3. School Office staff scans and uploads the form into Infinite Campus on the Student's Census > Relationship > Documents tab. Add/modify the student relationship for the contact indicated, checking Mailing, Portal and Messenger if applicable to the request. (See example picture below)


Examples:

  • School official release of medical records to a provider
  • Step-Parent requests Parent Portal access and Messenger emails
  • Grandparent requests Report Cards via Parent Portal access

Special Education Surrogate Form (Appendix II)
  1. Parent/guardian of a special education student requests appointing a surrogate.
  2. A parents/guardian completes SURROGATE PARENT REQUEST Form OR some documentation (a signed and dated note) providing educational decisions to another. At least one parent/guardian signature required on the completed form/document.
  3. School Office staff scans and uploads the form into Infinite Campus on the Student's Census > Relationship > Documents tab. Add/modify the student relationship for the contact indicated, checking Guardian, Mailing, Portal and Messenger if applicable to the request. (See example picture below)


Examples:

  • Student representative for special education when parents unavailable.

Delegation of Parental Authority / Power-of-Attorney Form (Appendix III)

  1. District requests parent/guardian to provide documentation that designates decision-making powers to someone else. Using this form negates the need to complete a Release form.
  2. A custodial parent/guardian of the student completes ELK RIVER PUBLIC SCHOOLS POWER OF ATTORNEY / DELEGATION OF PARENTAL AUTHORITY Form. All custodial parent/guardians signatures are required on the completed form; however a building administrator can sign off if one of the parent/guardian is unavailable for signature.
  3. School Office staff scans and uploads the form into Infinite Campus on the Student's Census > Relationship > Documents tab. Add/modify the student relationship for the contact indicated, checking Guardian, Mailing, Portal and Messenger if applicable to the request. (See example picture below)


Examples:

  • Student moves in with grandparent, or anyone other than parents.
  • Non-custodial parent pickup from school
  • Non-custodial parent consent for medical treatment


  • Students who are 18 years or older optionally may complete an STUDENT AGE WAIVER form to deny custodial parent/guardian access to their student records.
